Google Ads for rehab & behavioral health: LSA, PPC, and the LegitScript path (2026)

Updated August 2026 · 10 min read · By Carlos Meza

Google Ads is still the single highest-intent channel for filling beds. In 2026, treatment center CPCs run $50-$300 for competitive keywords, cost per lead lands at $150-$350, and cost per admit typically comes in between $2,500 and $8,000. To advertise at all, you need LegitScript certification — that's non-negotiable. Local Services Ads (LSA) is NOT available for addiction treatment in most markets as of 2026, so you're running standard Search and Display. This guide is how to actually make Google Ads work for a rehab without burning $50,000 to figure it out yourself.

Written for owners and marketing leads at treatment centers who are either about to spin up Google Ads for the first time or watching an existing campaign underperform and wondering why.

The three flavors of Google Ads for rehab (and which ones actually work)

1. Google Search Ads — your workhorse

This is where the money is. Someone types "rehab near me" or "inpatient detox Miami" and your ad shows above the organic results. Highest intent, highest cost, highest admit rate. If you're only going to run one thing, run Search.

2. Google Display / YouTube — for retargeting only

Display ads work for hitting people who already visited your site. Do NOT use Display for cold prospecting in behavioral health. Cold Display in this niche produces almost zero admits — the intent isn't there, the audience quality is bad, and you'll burn budget fast. Retargeting Display (people who came to your site and didn't convert) is legitimate and cheap. Cold Display is a money pit.

3. Local Services Ads (LSA) — mostly not available for rehab

LSA shows those lead-form ads with the green Google Guaranteed badge above regular search results. It's amazing for HVAC, plumbing, and locksmiths. For rehab, it's not eligible in most markets as of 2026. Google specifically excludes addiction treatment and mental health from LSA in the US. Some behavioral health adjacent categories (therapy, counseling) have limited LSA availability in select cities but full inpatient/residential rehab does not qualify. Don't waste time trying to make LSA work — put the budget in Search.

Bottom line: for a treatment center in 2026, "Google Ads strategy" really means "Google Search Ads strategy" with retargeting Display as a backup. Don't get distracted by other Google surfaces.

The LegitScript step (do this before you spend a dollar)

You cannot run addiction treatment ads on Google without LegitScript certification. That's Google's policy since 2017 and it's enforced strictly. If you don't have LegitScript, stop reading this and go read the LegitScript compliance guide first. Takes 6-12 weeks to get certified, so plan ahead.

Once you're certified, you register your Google Ads account with LegitScript, which flags it as approved on Google's side. Ads can start running within 24-48 hours of that registration.

Behavioral health that's NOT addiction (therapy, counseling, anxiety, depression treatment without SUD component) sometimes runs without LegitScript — but the line has been moving and Google's been strict lately. If you offer any SUD services at all, get certified.

What Google Ads actually costs a rehab in 2026

MetricTypical rangeNotes
CPC (cost per click)$50-$300+Competitive terms in FL/CA hit $200-$300. Smaller markets $50-$100.
Cost per lead (form fill / call)$150-$350Below $150 usually means bad-quality leads or wrong intent.
Lead-to-admit rate10-20%Below 10% means your admissions team or landing page is broken.
Cost per admit$2,500-$8,000Under $3K is exceptional. Over $8K means fix your funnel.
Minimum viable monthly budget$8,000-$12,000Below this, you can't get enough data to optimize.

Real number to run: figure out your average revenue per admission (residential 30-day: $15K-$30K; PHP: $8K-$15K; IOP: $3K-$8K). A $5,000 cost per admit on a $22,000 residential admit is a 4.4x return. That's the target.

The keyword categories that matter

High-intent commercial keywords (bid aggressively)

Mid-intent (bid conservatively, test carefully)

Info-seeker keywords (add to negative list)

These will burn your budget on zero admits. Add every one of them to your negative keywords list on day one:

Negative keyword hygiene is the single highest-ROI activity in a Google Ads account. Every week, pull your search terms report and add anything irrelevant to your negatives. Do this from week 1.

Campaign structure that works

Don't build one giant campaign. Split by intent and by level of care so you can control budget and messaging separately.

  1. Campaign 1 — Branded search: your rehab's name + variations. Cheap, high converting, defends against competitors bidding on your brand. Always on.
  2. Campaign 2 — High-intent geo: "rehab near me" + city-specific queries. Highest priority, biggest budget, tight ad groups by city.
  3. Campaign 3 — Level-of-care: "IOP [city]", "PHP [city]", "detox [state]" — separated by service line because CPCs and conversion rates differ meaningfully.
  4. Campaign 4 — Insurance queries: "BCBS covered rehab", "Aetna addiction treatment", etc. Requires landing pages that immediately confirm insurance acceptance.
  5. Campaign 5 — Substance-specific: alcohol, opioid, meth queries. Split when volume justifies (usually after month 2).
  6. Campaign 6 — Family / decision-maker: "how to help someone with addiction", "family intervention", etc. Longer sales cycle, warmer audience, valuable for filling residential beds.

The landing page problem (where most rehabs leak money)

You spend $200 to get a click. That click hits your homepage. Your homepage has 8 things on it — services, staff, testimonials, blog, contact form buried in the footer. The visitor takes 4 seconds, gets confused, leaves.

You just paid $200 for nothing.

Every ad campaign needs a dedicated landing page. Not your homepage. A single-purpose page that:

A rehab with a decent Google Ads campaign and a bad landing page will pay $500+ per lead. A rehab with the same campaign and a good landing page will pay $150-$200. Same clicks, 3x the conversions.

The mistakes that burn budget fastest

1. Running one campaign with 40 keywords in one ad group

Google can't optimize when everything's mashed together. Split by intent. Split by city. Split by level of care. More structure, better performance.

2. Bidding on info-seeker keywords as if they were buyers

Someone searching "signs of alcohol addiction" isn't a buyer. They're doing research. Every click costs you $50-$150 and produces zero admits. Add to negatives immediately.

3. Not running call tracking

If you can't tell which keyword drove which call, you can't optimize. Install CallRail or Invoca on day one. $50-$150/mo. Non-negotiable.

4. Sending all traffic to the homepage

Covered above. Dedicated landing pages per campaign. Every time.

5. Not tracking beyond the lead

Google optimizes for whatever conversion you tell it to. If you optimize for "form submitted", Google gets you cheap form submissions from tire-kickers. If you optimize for "qualified lead" (form + insurance verified) or "admission booked", Google gets you fewer but MUCH better leads. Send offline conversion data back to Google Ads. This is the single biggest algorithmic lever you have.

6. Ignoring your admissions team's feedback

Your admissions counselors know which leads are junk within 30 seconds of the call. Ask them weekly which campaigns produce garbage. Pause those. Double down on the ones producing real admits.

Running Google Ads and not seeing admissions?

We audit rehab Google Ads accounts and typically find $3,000-$8,000/mo of budget being wasted on the wrong keywords, wrong landing pages, or missing tracking. Free 45-minute audit call.

How long until you see admissions

If everything is set up right — LegitScript approved, landing pages built, tracking in place, campaign structure clean — here's the realistic timeline:

Anyone who promises admissions "in the first week" is either lying or setting you up to accept low-quality leads. Real optimization takes 60-90 days.

What "good" looks like for a rehab Google Ads program

If you're missing three or more of these, your account has real leaks that are costing you $5,000-$15,000/mo in wasted spend. Fix them.

Frequently asked questions

How much do Google Ads cost for a rehab center?

CPCs run $50-$300+ for competitive terms. Cost per lead $150-$350. Cost per admit typically $2,500-$8,000. South Florida and California are the most competitive markets. Minimum viable monthly budget is $8,000-$12,000 to get enough data for optimization.

Do I need LegitScript to run Google Ads?

Yes. Google requires LegitScript Addiction Treatment Certification for any ad promoting addiction, mental health, or behavioral health services. Without it, your ads get disapproved and your account gets flagged. Read the LegitScript compliance guide for the full process.

What is Local Services Ads (LSA) and does rehab qualify?

LSA is the lead-form ad format with the Google Guaranteed badge above search results. As of 2026, addiction treatment and mental health are NOT eligible for LSA in most US markets. Standard Google Search Ads is the path for treatment centers.

How long until I see admissions from Google Ads?

First qualified inquiries in 7-14 days if you're LegitScript-approved with landing pages ready. First actual admissions in 3-6 weeks. Stable, predictable cost per admit takes 60-90 days of optimization.

What is the biggest mistake rehabs make with Google Ads?

Running informational keywords like "signs of alcohol addiction" or "what is rehab like" as if they were commercial. Those queries are researchers, not buyers. Bid on high-intent commercial queries: "rehab near me", "inpatient rehab [city]", insurance-specific like "BCBS covered rehab". Info-seeker keywords are the fastest way to burn $10,000/month on zero admissions.

Should I hire a rehab-specialist Google Ads agency or a generalist?

Specialist. Rehab Google Ads has LegitScript-specific quirks, behavioral health ad policy landmines, and keyword taxonomy that generalists don't know. A generalist agency will make expensive mistakes in the first 90 days. Expect to pay $3,500-$8,000/month for a specialist agency, and don't hire anyone who won't share past client results at scale.
